Der RIASON anti-hTg c.t. Kit ist ein coated tube Radioimmunoassay zur
quantitativen Bestimmung der Autoantikörper gegen Thyroglobulin (Tg) in
Humanserum im Bereich von 30-3000 IU/mL, unter Verwendung von 100 µL
Serumproben. Jeder Kit enthält ausreichend Material für 100 Assay Röhrchen und
erlaubt die Erstellung einer Standardkurve und den Assay von 42 unbekannten
Proben in Doppelbestimmung.
Die Testergebnisse sollten in Verbindung mit anderen klinischen oder
labordiagnostischen Daten verwendet werden, um den Arzt bei der Beurteilung einer
Schilddrüsenfunktionsstörung zu unterstützen.
Einleitung
Das menschliche Thyroglobulin (hTg) ist ein schweres Glycoprotein (605 kDa), das in
Thyreoideafollikelzellen zu finden ist. Es spielt eine zentrale Rolle in der Aufnahme
und Inkorporation, sowie der regulierten Biosynthese der Schilddrüsenhormone T4

Schilddrüsenstörungen haben großteils einen autoimmunen Ursprung und anti-Tg Ak sind die ersten Anzeichen, die darauf hindeuten. Anti-hTg kann in allen Schilddrüsenautoimmunerkrankungen, wie beispielsweise Hashimoto Thyreoiditis und Morbus Basedow (Graves' disease), nachgewiesen werden, wobei die höchsten Konzentrationen bei Hashimoto Thyreoiditis auftreten. Anti-hTG Ak können auch bei Schilddrüsenkrebs charakteristisch sein. Deren Bestimmung kann zur Verlaufskontrolle bei Krebspatienten benutzt werden. Das Bindeverhalten der anti-hTg Ak ist komplex, die Proben von unterschiedlichen Patienten zeigen variable Reaktivitäten zu Epitopen des hTG Moleküls. Messprinzip Der RIASON anti-hTg c.t. Kit ist ein kompetitiver Immunoassay, der mit hTg Antikörper beschichtete Polystyrol Röhrchen verwendet. Im Test konkurrieren die hTg Antikörper der Probe mit dem, am Kunststoffröhrchen immobilisierten, Antikörper um die Bindung an den, mit 125I-markierten, hTg-Tracer. Nicht am Röhrchen gebundenes 125I-markiertes hTg wird durch den Waschschritt aus dem Röhrchen entfernt. Die Präsenz von hTg Antikörpern im gemessenen Test verursacht eine reduzierte Bindung des 125I-markierten Tg. Somit ist die Antikörperkonzentration des Tests umgekehrt proportional zu der im Röhrchen gebundenen Radioaktivität. Warnhinweise und Vorschichtsmaßnahmen Der RIASON anti-hTg c.t. Kit ist nur zur in-vitro Diagnostik und nicht zur Anwendung bei Mensch und Tier geeignet. Dieses Produkt darf nur exakt wie in der mitgelieferten Anleitung beschrieben, eingesetzt werden. IASON kann nicht für einen eventuellen Verlust oder Schaden, (sofern nicht gesetzlich etwas anderes festgelegt ist), haftbar gemacht werden, der durch Nichtbeachtung der Instruktionen entsteht. VORSICHT: Dieser Kit enthält Material menschlichen und/oder tierischen Ursprungs. Die Kitreagenzien sind so zu handhaben, als ob eine Übertragung einer Infektionskrankheit möglich sei. Rohmaterial humanen Ursprungs, das in diesem Kit zum Einsatz kommt ist negativ auf HBsAg, HCV und HIV-Antikörper geprüft. Da es kein diagnostisches Verfahren gibt, welches derartige Erreger mit 100%iger Sicherheit ausschließt, sind die entsprechenden Komponenten wie potentiell infektiöse Proben handzuhaben. Angemessene Vorsichtsmaßnahmen und die Regeln der guten Laborpraxis sollten beim Gebrauch und der Entsorgung angewendet werden. Die Entsorgung der Kitreagenzien hat nach den örtlichen Vorschriften zu erfolgen. Beschädigte Testkits Im Falle einer starken Beschädigung des Testkits oder der Komponenten muss die Firma IASON in schriftlicher Form spätestens eine Woche nach Erhalt des Kits informiert werden. Stark beschädigte Einzelkomponenten sollten nicht für den Testlauf verwendet werden. Sie müssen gelagert werden, bis eine endgültige Lösung gefunden wurde. Haltbarkeit und Lagerung der Reagenzien
Dieser Kit ist bei vorschriftgemäßer Lagerung bis zum Ablauf des Verfallsdatum
stabil. Nach Erhalt sind alle Reagenzien bei 2-8° C zu lagern.
Alle Reagenzien mindestens 20 Minuten vor Arbeitsbeginn bei Raumtemperatur
lagern.
Lagerung und Vorbereitung der Patientenproben
Die Proben sollen so rasch wie möglich nach der Abnahme getrennt und innerhalb
24 Stunden analysiert oder bei unter –20° C gelagert werden. Wiederholtes
Einfrieren und Auftauen ist zu vermeiden ggfs. sollte gleich aliquotiert eingefroren
werden.
Keine lipämische und stark hämolytische Patientenproben verwenden.
Keine Plasmen verwenden.
Wenn erforderlich, die Patientenproben bei Raumtemperaturen auftauen lassen und
gut vortexen.
Das Serum sollte vor dem Test noch einmal zentrifugiert werden, um die Partikel zu
entfernen (5 min bei 10-15 000 x g). Bitte diesen Schritt nicht auslassen, wenn die
Proben trüb sind oder Partikel erkennbar sind.
Kitbestandteile
Alle Reagenzien 1 – 6 vor Gebrauch auf Raumtemperatur bringen.
1. TRAC 125I hTg-Tracer (1 x 22 mL); max. 260 kBq radioactive ;
gebrauchsfertig; mit 0,1% NaN3;
2. CT Coated tubes (2 x 50 Röhrchen, 12x75 mm, beschichtet mit anti-hTg
Antikörper) gebrauchsfertig;
3. CAL0-CAL5 (je 0,75 mL; gelb); gebrauchsfertig; mit 0,1% NaN3;
Konzentrationen: 0, 30, 100, 300, 1000, 3000 IU/mL;
4. CO1, CO2 (je 0,75 mL; gelb); gebrauchsfertig; mit 0,1% NaN3;
Konzentrationen siehe QC-Zertifikat.

Testdurchführung
Allgemeine Hinweise
Alle Reagenzien auf Raumtemperatur bringen. Die Anzahl der erforderlichen
Teströhrchen berechnen.
CAL0-CAL5, CO1,CO2 und die Patientenproben sollten in Doppelbestimmungen
durchgeführt werden.
2 unbeschichtete Röhrchen für die Messung der Totalaktivität bereitstellen.
Testdurchführung
1. 100 µL CAL0-CAL5, CO1,CO2 und Patientenproben in die Röhrchen
pipettieren
2. 200 µL TRAC in jedes Röhrchen pipettieren (Totalaktivität nicht vergesen).
3. 2 Stunden bei Raumtemperatur (20-25°C) auf einem Schüttler (min. 600 UpM)
inkubieren.
4. 1 mL destilliertes Wasser in jedes Röhrchen pipettieren, absaugen/
dekantieren und für 2 Minuten überkopf auf Papier abtropfen lassen.
5. Jedes Röhrchen einschließlich Totalaktivität 1-2 Minuten im RIA-Counter
messen.
Ergebnisberechnung
Eine Standardkurve anlegen, indem man die Counts per minutes (CPM)
der einzelnen Standards gegen die Konzentration aufträgt. So können die
Konzentrationen der Patientenproben abgelesen werden.
Alternative Auswertemethoden können verwendet werden, allerdings sollte
bestätigt werden, dass der gewählte Kurvenverlauf geeignet ist und
akzeptable Ergebnisse liefert. Eine Spline-Auswertung wird empfohlen.
Proben mit hoher TgAb-Konzentration können 1:10 mit CAL0 verdünnt
werden. Weitere Verdünnungen können von dieser 10x Verdünnung
vorbereitet werden. Einige Seren lassen sich nicht linear verdünnen.

Jedes Labor sollte seinen eigenen Normalbereich für die lokale Bevölkerung
festlegen. Eine klinische Diagnose sollte nicht allein mit Hilfe der Ergebnisse einer
einzelnen diagnostischen Methode erhoben werden. Die Ergebnisse sollten mit
anderen klinischen Befunden und Observationen korrelieren.
Qualitätskontrolle
Es wird empfohlen, die Kontrollproben gemäß den nationalen gesetzlichen
Bestimmungen einzusetzen.
Durch die Verwendung von Kontrollproben wird eine Tag-zu-Tag Überprüfung der
Ergebnisse erzielt. Es sollten Kontrollen sowohl mit normalem als auch
pathologischem Level eingesetzt werden.
Die Kontrollen mit den entsprechenden Ergebnissen des IASON QC-Labors sind im
QC-Zertifikat, das dem Kit beiliegt, aufgeführt. Die im QC-Blatt angegebenen Werte
und Bereiche beziehen sich stets auf die aktuelle Kitcharge und sollten zum direkten
Vergleich der Ergebnisse verwendet werden.
Es wird ebenfalls empfohlen, an nationalen oder internationalen Qualitätssicherungs-
Programmen teilzunehmen, um die Genauigkeit der Ergebnisse zu sichern.
Es sollten geeignete statistische Methoden zur Analyse von Kontroll-Werten und
Trends angewendet werden. Wenn die Ergebnisse des Assays nicht mit den
angegebenen Akzeptanzbereichen des Kontrollmaterials übereinstimmen, sollten die
Patientenergebnisse als ungültig eingestuft werden.
In diesem Fall überprüfen Sie bitte die folgenden Bereiche: Pipetten und Zeitnehmer,
Gamma Counter, Verfallsdatum der Reagenzien, Lagerungs- und
Inkubationsbedingungen, Absaug- und Waschmethode.
Sollten Sie nach Überprüfung der vorgenannten Bereiche keinen Fehler erkannt
haben, setzen Sie sich bitte mit Ihrem Lieferanten oder direkt mit der Firma IASON
GmbH in Verbindung.
Test Charakteristika
Messbereich
Der Messbereich des Testes liegt zwischen 30 und 3000 IU/mL.
Standards sind gegen den Internationalen Referenz Standard NIBSC 65/93 kalibriert.

Analytische Sensitivität
Die analytische Sensitivität definiert als Mittelwert minus der zweifachen
Standardabweichung des CAL0 (n = 20) beträgt 8,6 IU/mL.
Funktionelle Sensitivität
wurde bei 13 IU/mL gefunden.
Präzision
Sieben Humanseren Pools wurden in 20 Wiederholungen getestet um eine Intra-
Assay Präzision zu bestimmen. Die Ergebnisse wurden in der Tabelle unten gezeigt.
Um eine Iner-Assay Präzision zu bestimmen, sieben Humanseren Pools wurden in
Duplikat in 20 unabhängigen Assays von 2 Operatoren mithilfe unterschiedlichen Kit-
Chargen gemessen. Die Ergebnisse wurden in der Tabelle unten gezeigt.

Linearität
Zwei Patienten-Serum wurden schrittweise mit dem CAL0 verdünnt und anti-Tg Ak
wurden determiniert. Die Wiederfindungsrate wurde von dem erwarteten Wert der
Duplikat-Determinationen festgelegt. Die Mittelwerte von zwei Patienten-Proben
waren 100 und 126 %.

Bezug auf Epitopen und Affinität, manche Proben können nicht linear verdünnt.

Grenzen des Tests
Es werden verlässliche und reproduzierbare Ergebnisse mit dem Test erzielt wenn
man sich strikt an die Arbeitsabarbeitung laut Kitanleitung hält und umsichtig mit
guter Laborpraxis arbeitet. Jede unsachgemäße Behandlung von Proben oder
Modifikationen dieses Tests können die Ergebnisse beeinflussen.

Rechtliche Grundlagen
Zuverlässigkeit der Ergebnisse
Der Test muss exakt gemäß der Testanleitung des Herstellers abgearbeitet werden. Darüber
hinaus muss der Benutzer sich strikt an die Regeln der GLP (Good Laboratory Practice) oder
andere eventuell anzuwendende Regeln oder nationale gesetzliche Vorgaben halten. Dies
betrifft besonders den Gebrauch der Kontrollreagenzien. Es ist sehr wichtig, bei der
Testdurchführung stets eine ausreichende Anzahl Kontrollen zur Überprüfung der
Genauigkeit und Präzision mitlaufen zu lassen. Die Testergebnisse sind nur gültig, wenn alle
Kontrollen in den vorgegebenen Bereichen liegen, und wenn alle anderen Testparameter die
vorgegebenen Spezifikationen für diesen Assay erfüllen. Wenn Sie bezüglich eines
Ergebnisses Zweifel oder Bedenken haben, setzen Sie sich bitte mit der Firma IASON
GmbH in Verbindung.
Therapeutische Konsequenzen
Therapeutische Konsequenzen sollten keinesfalls nur aufgrund von Laborergebnissen
erfolgen, selbst dann nicht, wenn alle Testergebnisse mit den in „Zuverlässigkeit der
Ergebnisse“ genannten Voraussetzungen übereinstimmen. Jedes Laborergebnis ist nur ein
Teil des klinischen Gesamtbildes eines Patienten. Nur in Fällen, in denen die
Laborergebnisse in akzeptabler Übereinstimmung mit dem allgemeinen klinischen Bild des
Patienten stehen, sollten therapeutische Konsequenzen eingeleitet werden. Das
Testergebnis allein sollte niemals als alleinige Grundlage für die Einleitung therapeutischer
Konsequenzen dienen.
Haftung
Jegliche Veränderungen des Testkits und/oder Austausch oder Vermischung von
Komponenten unterschiedlicher Chargen von einem Testkit zu einem anderen, können die
gewünschten Ergebnisse und die Gültigkeit des gesamten Tests negativ beeinflussen.
Solche Veränderungen und/oder Austausch haben den Ausschluss jeglicher
Ersatzansprüche zur Folge. Reklamationen, die aufgrund von Falschinterpretation von
Laborergebnissen durch den Kunden gemäß Punkt „Therapeutische Konsequenzen“
erfolgen, sind ebenfalls abzuweisen. Im Falle jeglicher Reklamation ist die Haftung des
Herstellers maximal auf den Wert des Testkits beschränkt. Jegliche Schäden, die während

The RIASON anti-hTg c.t. kit is a coated tube radioimmunoassay intended for the
quantitative determination of autoantibodies against Thyroglobulin (Tg) in hunman
serum in the range of about 30-3000 IU/mL using 100 µL serum samples. Each kit
contains materials sufficient for 100 assay tubes permitting the construction of one
standard curve and the assay of 42 unknowns in duplicate.
Results are to be used in conjunction with other clinical and laboratory data to assist
the clinician in the assessment of thyroid dysfunction.

REF R01-020-100X Introduction The human thyroglobulin (hTG) is a high molecular weight glycoprotein (605 kDa) found in the thyroid follicular cells. It plays a central role in the uptake, incorporation, and regulated biosynthesis of thyroid hormones, T4 and T3. Thyroid disorders are, in large part, due to autoimmun origin, and anti-thyroglobulin autoantibodies were the first factor to be discovered. Anti-hTG is found in all thyroid autoimmune diseases (Hashimoto’s thyroiditis, Graves’ diseases), with the highest level observed in Hashimoto’s thyroiditis. Anti-hTG is also characteristic of thyroid cancer, and its determination can be used for the follow up of cancer patients. The specificity of anti-hTG autoantibodies is complex, and samples from different patients show a variable reactivity towards epitopes present in the hTG molecule. Assay principle The RIASON anti-hTg c.t. kit is a competitive immunoassay using polystyrene tubes coated with hTg antibodies. In the assay, hTg autoantibodies compete with the antibody immobilised on the tube for binding sites on 125I-labelled hTg-Tracer. Unbound labelled hTg is then removed from the tube by a wash step. The presence of hTg autoantibody in a test sample causes a reduction in the amount of labelled hTg bound to the tube and hence, the antibody concentration in the sample is inversely proportional to the radioactivity bound to the tube. Warnings and precautions The RIASON anti-hTg c.t. kit is for in vitro diagnostic use only and is not for internal use in humans or animals. This product must be used strictly in accordance with the instructions set out in the Package Insert. IASON will not be held responsible for any loss or damage (except as required by statute) caused, arising out of non-compliance with the instructions provided. CAUTION: this kit contains material of human and/or animal origin. Handle kit reagents as if capable of transmitting an infectious agent. Source material from Human origin which is used in this kit was tested and found negative for HbsAG and HIV as well as for HCV antibodies. However, since there is no diagnostic procedure that excludes these agents with 100 percent certainty all components should be handled as potentially hazardous material. Appropriate precautions and good laboratory practices must be used in the storage, handling and disposal of the kit reagents. Disposal of kit reagents should be in accordance with local regulations. Damaged test kit In case of serious damage to the test kit or components, the company IASON must be notified in writing at least one week after receiving the kit. Severely damaged single components should not be used for the test run. They must be stored until a final solution has been found. Shelf life and storage of reagents
This kit is stable until the stated expiry date if stored as specified.
Upon receipt, store all reagents at 2-8C.
Allow all reagents to reach room temperature at least 20 minutes before use.
Storage and preparation of serum samples
Sera to be analysed should be assayed within 24 hours after separation or stored,
preferably in aliquots, at or below -20°C. Repeated freezing and thawing or increases
in storage temperature must be avoided.
Do not use lipemic, grossly haemolysed or turbid serum samples.
Do not use plasma in the assay.
When required, thaw test sera at room temperature and mix gently to ensure
homogeneity.
Centrifuge the sera prior to assay (preferably for 5 min. at 10.000 -15.000 g in a
microfuge) to remove any particulate matter. Please do not omit this centrifugation

Assay procedure
General remarks
Calculate the number of tubes needed for the assay. Allow all the reagents supplied
including the tubes to reach room temperature. All CAL0-CAL5, CO1, CO2 and test
sera should be assayed in duplicate.
2 uncoated tubes should be used for measuring total counts.
Test procedure
1. Pipette 100 µL of CAL0-CAL5, CO1, CO2 and test sera into the tubes.
2. Pipette 200µL of TRAC into each tube (don’t forget the total activity)
3. Incubate for 120 min. at room temperature (20-25°C) on a shaker
(min. 600 rpm).
4. Add 1 mL of distilled water to each tube, then aspirate or decant and put it
2 minutes on absorbent material
5. Count each tube including the total count tubes of 125I for 1-2 minutes.
Calculation of results
Draw a standard curve by plotting the counts per minute (cpm) of each
standard against its concentration. Read off the values of the test samples.
Alternative data reduction techniques may be employed but users should
confirm that the selected curve fit is appropriate and gives acceptable
results. Spline curve fits are recommended.
Samples with high TgAb concentrations can be diluted 1:10 in CAL0. Other
dilutions can be prepared from a 10x dilution or otherwise as appropriate.
Some sera will not dilute in a linear way.

Each laboratory is recommended to determine ranges for their local population. The
results alone should not be the only reason for any therapeutic consequences. The
results should be correlated to other clinical observations and diagnostic tests
Quality control
The regular use of control samples at several analyte levels is advised to ensure day-
to-day validity of results. Controls should be tested as unknowns. Quality Control
charts should be maintained to follow the assay performance.
Good laboratory practice requires that controls be run with each calibration curve. A
statistically significant number of controls should be assayed to establish mean
values and acceptable ranges to assure proper performance.
It is recommended to use control samples according to state and federal regulations.
The use of control samples is advised to assure the day to day validity of results. Use
controls at both normal and pathological levels.
The controls and the corresponding results of the IASON QC-Laboratory are stated
in the QC certificate added to the kit. The values and ranges stated on the QC sheet
always refer to the current kit lot and should be used for direct comparison of the
results.
It is also recommended to make use of national or international Quality Assessment
programs in order to ensure the accuracy of the results.
Employ appropriate statistical methods for analysing control values and trends. If the
results of the assay do not fit to the established acceptable ranges of control
materials patient results should be considered invalid. In this case, please check the
following technical areas: Pipetting and timing devices; gamma counter, expiration
dates of reagents, storage and incubation conditions, aspiration and washing
methods.

Test characteristics
Assay dynamic range
The range of the assay is between 30 and 3000 IU/mL.
Standards are calibrated against the international reference standard NIBSC 65/93.

Analytical sensitivity
The analytical sensitivity was calculated from the mean of CAL 0 (n = 20) minus its
double standard deviations determined to be: 8.6 IU/mL.
Functional sensitivity
was found to be 13 IU/mL.
Reproducibility
Seven human serum pools were assayed in 20 replicates to determine intra-assay
precision. Values obtained are shown below.
To determine inter-assay precision 7 human serum pools were measured in
duplicates in 20 independent assays by 2 operators using different kit batches.

Linearity
Two patient serum samples were serially diluted with zero calibrator and anti-TG
values determined. The percent recovery from the expected value of duplicate
determinations was calculated. The mean recovery for two patient samples was
obtained 100 and 126 %.
Due to the inherent heterogeneity of the autoantibody population in human sera with
regard to epitopes and affinity, some samples may not dilute linearly.
Limitation of use
Reliable and reproducible results will be obtained when the assay procedure is
performed with a complete understanding of the package insert instruction and with
adherence to good laboratory practice. Any improper handling of samples or
modification of this test might influence the results.

Legal aspects
Reliability of results
The test must be performed exactly as per the manufacturer’s instructions for use.
Moreover the user must strictly adhere to the rules of GLP (Good Laboratory
Practice) or other applicable national standards and/or laws. This is especially
relevant for the use of control reagents. It is important to always include, within the
test procedure, a sufficient number of controls for validating the accuracy and
precision of the test.
The test results are valid only if all controls are within the specified ranges and if all
other test parameters are also within the given assay specifications. In case of any
doubt or concern please contact IASON.
Therapeutic consequences
Therapeutic consequences should never be based on laboratory results alone even if
all test results are in agreement with the items as stated under Reliability of Results.
Any laboratory result is only a part of the total clinical picture of a patient.
Only in cases where the laboratory results are in acceptable agreement with the
overall clinical picture of the patient should therapeutic consequences be derived.
The test result itself should never be the sole determinant for deriving any therapeutic
consequences.
Liability
Any modification of the test kit and/or exchange or mixture of any components of
different lots from one test kit to another could negatively affect the intended results
and validity of the overall test. Such modification and/or exchanges invalidate any
claim for replacement.
Claims submitted due to customer misinterpretation of laboratory results subject to
point Therapeutic Consequences are also invalid. Regardless, in the event of any
claim, the manufacturer’s liability is not to exceed the value of the test kit. Any
damage caused to the test kit during transportation is not subject to the liability of the
manufacturer.
